The undersigned owner and/or officer of the above named Entity represent and warrant that the information provided on this form is accurate and complete, and further agree to the following terms: The services provided hereunder by Agent shall commence as of the date below and upon receipt of full payment by agent. This agreement shall continue for the term of one (1) year from signature date indicated below.I further understand that fees paid for this service are considered fully earned and non-refundable. I may cancel this service at anytime prior to the expiration date and upon thirty (30) days written notice to Agent. Upon expiration, or cancellation, of this agreement, Agent shall the file appropriate form with the Secretary of State indicating resignation as such agent, unless a new agreement has been signed, the annual fees paid, and agreement has been accepted by Agent. I agree with the Entity, to be jointly and severally liable for all obligations hereunder, and further agree that all disputes regarding this agreement shall be settled by binding arbitration in the County of Ventura, California, and that the Agent’s liability hereunder shall not include any indirect or consequential damages, and damages therefore shall in no instance exceed the refund of any amounts paid hereunder.
